Before your first visit, please check your email inbox for a link to our secure online intake forms.  Note that this email will come from charmtracker.com, not vitaehealthcenter.com, so you may have to check your spam folder.  The provided link allows you to securely fill out the forms online at your own pace.   Please take your time when filling out the intake, as this information enables your doctor to gain valuable insight and start working for you before your first visit!

It is very important to have the forms completed before your first appointment. If you are unable to complete them please let us know and we can reschedule your appointment to a later date.

Once arriving at Vitae Health Center, if we are not at the front desk please make yourself comfortable, as you may experience a short wait. You are welcome to take some time to browse our educational materials by checking out the library of magazines, books and educational articles. While perusing the educational materials feel free to help yourself to our incredible local artesian well water and herbal tea.

 

We value your time at Vitae Health Center and make it a priority to stay on time unless an urgent or emergency situation requires extra time to be spent with other patients.

(Initial appointments can last up to 60 minutes)

    A review of your intake forms and any associated past lab work

    Comprehensive medical and lifestyle history

    Review of medications and supplements that you are currently taking

    Appropriate physical exam

    Detailed explanation of our analysis and approach to your case

    Individualized comprehensive lifestyle recommendations & treatment plan
